Fees and payment:

Applicable fees and payment are outlined in the invoice provided by Create Vitality.

Privacy and confidentiality:

As part of the agreement Create Vitality may obtain personal details or confidential material relating to the client.  Create Vitality undertakes to maintain strict levels of confidentiality with all client information. There are however conditions, by law, under which confidentiality may not be maintained.  This includes but is not limited to illegal activity, pursuant to a valid court order or subpoena, imminent or likely risk of danger to self or others.  Create Vitality will seek to discuss issues relating to confidentiality with the client prior to third party sharing, unless circumstances prohibit this.

Client Responsibilities and Acknowledgements:

The client accepts and agrees that the client is responsible for their progress and results in the program. Create Vitality makes no representations, warranties or guarantees verbally or in writing. The client understands that because of the nature of the program, results experienced by each client may vary.

The client is solely responsible for creating and implementing their own physical, mental and emotional well-being, decisions, choices, actions and results arising out of or resulting from engaging the services of Create Vitality.  As such, the Client agrees that Create Vitality is not and will not be liable or responsible for any actions or inaction, or for any direct or indirect result of any services provided by Create Vitality.  

The client acknowledges that Create Vitality does not provide diagnosis or treatment of mental disorders as defined by the American Psychiatric Association and/or equivalent body in their country and that the program is not to be used as a substitute for counselling, psychotherapy, psychoanalysis, mental health care, substance abuse treatment or other professional advice by legal medical or other qualified professionals and that it is the client’s exclusive responsibility to seek such independent professional guidance as needed.

No Transfer of Intellectual Property:

Create Vitality program materials are copyrighted and materials provided to the client are for the individual client’s use only. No licence to sell or distribute Create Vitality’s materials and programs is granted or implied.
